Upon selecting “yes” I grant Glendo LLC permission to use supplied materials such as, but not limited to, drawings, photographs, and / or videos of me and my work, and / or to take photographs and / or videos of me and my work for use on its website(s), digital or printed advertisements, or in other printed publications without further consideration. I grant permission to Glendo LLC to use or alter the photographs or videos at its discretion. I also acknowledge that Glendo LLC owns the rights to any photographs or videos taken by its employees. I understand that I retain all legal rights to my supplied materials, and that I am granting permission of use in any manner at the discretion of and by Glendo LLC, which will respect my private information and not disclose it to a third party.

If I select “no” I understand that Glendo employees will make every effort to respect my privacy while in class. I understand that I may be asked to participate in the class photograph, which will be released to this class only and not used for other purposes, and that I may opt out or choose to participate at my discretion.

If I am signing someone else up for a course , I understand that the person attending this class needs to fill the form out prior to attending class.
